Preliminary Exams: Three weeks beginning Monday, October 1, and ending Friday, October 19, (or Friday October 26, if the alternate dates are needed);

Fall Break begins 7:00 p.m., Friday October 5, and ends 8:30 a.m., Wednesday, October 10.

This form is used by students to request cash advances for business travel and other business expenses. The money received as an advance must be accounted for within 30 days of the trip's end by submitting a Travel Expense Form. The advance will appear on the student's Bursar Account until cleared by the Expense Report

Personal Data Change Form (DOC)
